Abstract

º» ¿¬±¸´Â ¼Ò±Ô¸ð »ç¾÷Àå º¸°Ç°ü¸® ÇÁ·Î±×·¥ÀÌ °í·É±Ù·ÎÀÚÀÇ °Ç°­¼öÁØ¿¡ ¹ÌÄ¡´Â È¿°ú¸¦ ÆľÇÇϱâ À§ÇØ ½Ç½ÃµÇ¾úÀ¸¸ç, 1°³ ¹Î°£ »ê¾÷º¸°ÇÀü¹®±â°ü¿¡¼­ 2009³â 3¿ùºÎÅÍ 11¿ù±îÁö ¹æ¹®ÇÑ »ç¾÷Àå Áß 253°³¼Ò¸¦ ´ë»óÀ¸·Î º¸°Ç°ü¸® ÇÁ·Î±×·¥À» Á¦°ø¹ÞÀº ±Ù·ÎÀÚ Áß 50¼¼ ÀÌ»ó °í·É±Ù·ÎÀÚ 608¸íÀ» ´ë»óÀ¸·Î ½Ç½ÃµÇ¾ú´Ù. ¿¬±¸´ë»óÀÚ Áß 77.6% ³²¼ºÀ̾ú°í, Æò±Õ¿¬·ÉÀº 62.05¡¾6.17¼¼ ¿´´Ù. 60¼¼ ÀÌ»ó 70¼¼ ¹Ì¸¸ ¿¬·ÉÃþÀÌ 55.9% Â÷ÁöÇÏ¿´´Ù. »ç¾÷ÀåÀÇ ±Ô¸ð´Â »ó½Ã±Ù·ÎÀÚ°¡ 5ÀÎ ÀÌÇÏÀÎ °æ¿ì°¡ 96.4% ¿´´Ù. ´ë»óÀÚ´Â 50¼¼ ÀÌ»ó °í·É±Ù·ÎÀÚ·Î Ç÷¾Ð, Ç÷´ç, ÃÑÄÝ·¹½ºÅ×·Ñ, ºñ¸¸µµ Áß ÇÑ °¡Áö ÀÌ»ó ÀÌ»ó¼Ò°ßÀÌ ÀÖ´Â ÀÚ·Î ¼±Á¤ÇÏ¿´°í, º¸°Ç±³À°, °£ÀÌ°ËÁø, °Ç°­»ó´ã, Á¤º¸Á¦°ø µîÀÇ º¸°Ç°ü¸®ÇÁ·Î±×·¥À» Á¦°øÇÏ¿´´Ù. ¿¬±¸°á°ú ¿¬±¸´ë»óÀÚÀÇ »ýÈ°½À°ü º¯È­´Â ±Ý¿¬°ú ¿îµ¿¿¡¼­ À¯ÀÇÇÑ ¼öÁØÀ¸·Î(<.0001) º¯È­µÇ¾ú°í, ½ÅüÁö¼ö º¯È­´Â ¼öÃà±â Ç÷¾ÐÀÇ °æ¿ì Æò±Õ 6.46mmHg °¨¼ÒÇÏ¿´°í, À̿ϱâ Ç÷¾ÐÀº Æò±Õ 5.22mmHg °¨¼ÒÇÏ¿´´Ù. Ç÷´çÀº 5.91mg/dL °¨¼ÒÇÏ¿´°í, ÃÑÄÝ·¹½ºÅ×·ÑÀº Æò±Õ6.35mg/dL °¨¼ÒÇÏ¿´À¸¸ç, ºñ¸¸µµ´Â Æò±Õ 0.31kg/m2 °¨¼ÒÇÏ¿´´Ù. ÇÁ·Î±×·¥ÀÇ È¿°ú¿Í »ýÈ°½À°ü ¹× ½ÅüÁö¼öÀÇ °ü·Ã¼ºÀ» ºÐ¼®ÇÑ °á°ú ÇÁ·Î±×·¥ ÀÚü È¿°ú°¡ ¼öÃà±âÇ÷¾Ð, À̿ϱâÇ÷¾Ð, Ç÷´ç, ÄÝ·¹½ºÅ×·ÑÀÇ °¨¼Ò¿Í °ü·Ã¼ºÀÌ ³ôÀº °ÍÀ¸·Î È®ÀεǾú´Ù. ƯÈ÷ ¼öÃà±âÇ÷¾Ð 160mmHg ÀÌ»óÀÇ °æ¿ì Ç÷¾Ð °¨¼Ò È¿°ú°¡ °¡Àå Å©°Ô ³ªÅ¸³µ°í, Ç÷´çÀº °øº¹½Ã Ç÷´ç 126mg/dL ÀÌ»óÀÇ Áý´Ü¿¡°Ô¼­, ÃÑÄÝ·¹½ºÅ×·ÑÀº 200mg/dLÀÌ»óÀÇ Áý´Ü¿¡°Ô¼­ Ç÷´ç°ú ÃÑÄÝ·¹½ºÅ×·ÑÀÇ °¨¼Ò È¿°ú°¡ À¯ÀÇÇÏ°Ô ³ô°Ô ³ªÅ¸³µÀ½À» È®ÀÎÇÏ¿´´Ù.

Purpose: The purpose of this study was to identify the effects of occupational health service(OHS) for the aged workers in small workplace.

Method: A single group of pretest- posttest design was used. 608 aged workers of 253 small workplace who had one out of 4 risk factors were randomly selected. The OHS consisted of education, medical checkup, and counseling. Also information was provided by a visiting nurse 4-5 times every 4-5 weeks from March to November, 2009. The data were collected by interview, self report, and monitored by a nurse.

Results: The OHS led to significant reduction of the mean systolic and diastolic blood pressures of 6.46mmHg and 5.22mmHg, each respectively. The mean total cholesterol, glucose, and BMI were also reduced significantly by 6.35mg/dL, 5.91mg/dL, and 0.31kg/m2. And stages of health behavioral change were moved from 1,2,3 stages to 4,5 stages significantly.

Conclusion: The OHS showed an effective intervention to improve the aged workers¡¯ health in the small workplaces. Therefore, The OHS by visiting a nurse can be a good strategy to extend to small & medium-sized companies having no in-site health manager.
